In pediatric medicine, child weight in kilograms is the single most critical determinant of therapeutic efficacy and safety. Age-based approximations on medication boxes are generalized estimates and frequently lead to sub-therapeutic underdosing (prolonging febrile discomfort) or dangerous accidental overdosing.
Target dose is 15 mg/kg per dose (therapeutic threshold 10–15 mg/kg). Minimum dosing gap is 4 hours; max 4 doses in 24 hours. Daily maximum ceiling is 60 mg/kg/day or 4,000 mg.

❓ How do I calculate the correct Paracetamol (Calpol / Crocin) dose for my child?
Pediatric paracetamol must always be calculated by weight (kg) rather than age. The clinical standard is 15 mg per kilogram of body weight per single dose (within a safe range of 10–15 mg/kg), given every 4 to 6 hours as needed. Never exceed 4 doses (or 60 mg/kg) in any 24-hour period.
❓ Can I give Paracetamol and Ibuprofen together to reduce fever faster?
Do not routinely combine or alternate paracetamol and ibuprofen without explicit instruction from your pediatrician. Alternating increases the risk of dosing confusion and accidental overdose. Paracetamol is the preferred first-line antipyretic. If fever remains high, consult Dr. Zahoor before adding ibuprofen.
❓ Is Ibuprofen safe for infants under 3 months?
No. Ibuprofen is strictly contraindicated for infants younger than 3 months or weighing under 5 kg. For infants between 3 and 6 months, ibuprofen should only be given under direct pediatric supervision. Always ensure the infant is well-hydrated before giving ibuprofen.

Kitchen teaspoons vary drastically in size (ranging anywhere from 2.5 mL to 8 mL). Using kitchen spoons is one of the leading causes of pediatric accidental poisoning and underdosing. Always use a calibrated oral syringe or medicine dropper supplied with the medicine.

If the child vomits immediately (within 10 minutes) and the entire liquid dose is visible, you may repeat the dose once.
